SAM 2021 is a hybrid event. This registration form is for those planning to attend IN PERSON.
The Florida Podiatric Medical Association is pleased to announce the annual Science & Management Symposium (SAM), taking place January 14 - 17, 2021 at Disney's Coronado Springs Resort, located in Lake Buena Vista, Florida. SAM's educational mission is to promote continuing education in podiatric medicine, podiatric surgery, and practice management through workshops and seminars. Our commitment is to increase knowledge by offering every registrant the educational assistance needed to improve care for their patients and be successful in practice by promoting the benefits of scientific advancement in the treatment, prevention, and alleviation of podiatric ailments.
Please Note: In order to register as an FPMA Member, you must register with the email address that is listed in the FPMA datebase.
If in-person registration has surpassed the four hundred attendee limit, please contact support@fpma.com if you would like to be added to a waitlist.

1) A separate registration form needs to be completed for each attendee.
2) All communication regarding SAM 2021 will be sent to the contact information provided on your registration form.
3) Your first and last name, as it appears on the registration form, will be printed on your SAM 2021 name badge.
4) You must be registered for the entire program to attend any portion of the program. Some workshops and events may require separate fees.
